Closing the Gap

The Closing the Gap program is aimed at reducing the gap in life expectancy between Aboriginal and Torres Strait Islander people and other Australians. The initiative is funded by the Australian Government Department of Health through the Indigenous Chronic Disease Package.

North Coast Primary Health Network (NCPHN) currently employs Indigenous Health Project Officers to work with Aboriginal Community Controlled Health Services, General Practices, Individual Doctors, Allied Health Services and Pharmacists to ensure their clients are engaged in their own health care.   • Working with practices to understand the scope of the Indigenous Health/Closing the Gap programs and what it means for their patients
  • Working with practices to register them for the for the Indigenous Practice Incentive Payment and the PBS Co-payment Scheme
  • Supporting practices to implement processes for registration of new patients for the Closing the Gap program and PBS co-payment measure
  • Informing practices of any changes to the Indigenous Practice Incentive Payment program
  • Providing practices with information on other programs and supports made available to practices and its Aboriginal and Torres Strait Islander patients
  • Informing practices of programs and supports available to Aboriginal and Torres Strait Islander patients

NCPHN also employs Aboriginal and Torres Strait Islander Outreach Workers to:

  • Provide assistance to Aboriginal and Torres Strait Islander patients to attend GP appointments
  • Support access to other health services including follow up care and visits to specialist services, allied health services and community pharmacies
  • Provide home visits and telephone support as required
  • Provide information on transport options
  • Assist Aboriginal and Torres Strait Islander patients with filling out forms and communicating with staff during health related appointments
