North Coast Primary Health Network’s 2017 Community Choice Excellence Award recognises a project or program that has engaged deeply with the community, and understands its characteristics and needs. The program should be aligned with the community’s principles and values, and the staff driving the program seen to act with sensitivity, respect and insight. The project is embraced by the community, and ultimately, empowers the community to take ownership of their own health and wellbeing.

On this page you will find four Community Choice Finalists, all of whom have been nominated as part of the 2017 Primary Health Care Excellence Awards judging process. These four finalists are committed to bringing better health and wellbeing to the North Coast community.
Please make your vote count.
Watch all four videos (each one is approximately 2.5 minutes long) and when you've finished watching, vote for your favourite program or service by clicking on the heart symbol. This will help North Coast Primary Health Network determine what kind of services the North Coast community wants.

The recipient of the 2017 Community Choice Award will be announced at the 2017 Primary Health Care Excellence Awards on Thursday 7 September 2017.
About the Primary Health Care Excellence Awards
The Primary Health Care Excellence Awards honour the individuals and teams working to bring better health to our region. They celebrate the incredible work carried out in the North Coast and inspire us to strive for greatness. The Awards also offer the opportunity to have some fun, which is what good health is about!
In 2017, we received 55 submissions. Those submitting had to follow a strict criteria, which included providing evidence of improved health for the North Coast community. Those who submitted included Community Organisations, Government and non-Government organisations, Allied Health, General Practice, Aged Care, Education and Aboriginal services.
The 2017 criteria was separated into four categories:
- Integration & Partnership
- Improving Health Care Access and/or Reducing Health Inequity
- Promoting Healthy Living
Judges, made up of community members, academics and health professionals, selected two Finalists from each category, plus one Community Choice Finalist from each category.
