General Practices and Aboriginal Medical Services of the Clarence Valley are invited to register their interest to participate in the 2017 Clarence Valley Integrated Care Collaborative (ICC).
What is the ICC?
The Northern NSW Local Health District (NNSWLHD), the North Coast Primary Health Network (NCPHN), Aboriginal Medical Services and NSW Ambulance are collaborating with General Practice through the Improvement Foundation to implement the Integrated Care Collaborative (ICC). After the success of Wave 1 in the Richmond/ Tweed area in 2015/16, Wave 2 is set to commence in the Clarence Valley in early 2017.
What will it achieve?
The ICC aims to improve health outcomes and prevent avoidable hospital admissions for people with complex, long term conditions. This will be achieved at a grass-roots level by General Practice and the NNSWLHD working together with patients and their families to deliver the right care, and empowering patients to be more active participants in their health management.
