End of Life Directions for Aged Care (ELDAC) is a free government-funded resource designed to connect you with evidence-based tools and resources in palliative care and advance care planning.

The ELDAC website offers a Home Care Toolkit developed by the University of Technology Sydney. The kit provides information and guidance to support palliative care and advance care planning for clients and their families. Within the toolkit you will find information, assessment tools, forms, templates and/or family resources.

Use the toolkit to find tools and resources at every step of care at the end of life or call the ELDAC Helpline (1800 870 155) for more information.
